This application is for both the Holiday Giving Tree and Food Box.

• Please read this application in its entirety before submitting it.
• To be eligible for this program you must be a current Crooked River Ranch resident.
• If only requesting a food box, only fill out the first 8 lines of the application
• If you are selected: you will be contacted by email confirming your selection
• You must be able to pick up the gifts and/or food box on Monday, December 23rd, between 8:00 a.m. and 6:00 p.m.
• Applications received after Friday, November 22nd at 5:00 pm, will not be accepted.

DIRECTIONS FOR APPLICATION FOR GIVING TREE & FOOD BOX

• You must list the age, gender, two needs, and one want per listed family member.
• Members include adults.
• If clothing list size and favorite color must be indicated in the request.
• Example 80-year-old woman. Needs: blanket (favorite color brown or tan), warm scarf (red). Wants: new playing cards.
